In young children the diagnosis of Asthma is based mainly on the clinical history.  It is expensive and extremely difficult to do lung function tests in very young children.

Children with coughing longer than three weeks and aggravated by playing, running, laughing, or crying has a high likelihood of having asthma if there is no fever, diarrhea, and poor weight gain.

Usually these children has had several “prolonged colds,” bronchitis, or pneumonia. Their parents had allergy, hay fever, eczema, or asthma.

Most clinicians make a diagnosis of asthma on clinical history and give a bronchodilator or steroids and observe. If there is improvement, asthma is the most likely problem.

If the coughing does not improve, a chest X-ray or other tests are requested to rule out other causes of chronic cough.
