A mother brought her 8 year old son to a doctor because of Fever.
Chief Complaint: Fever for 3 days.
History of Present Illness: My son has been having fever for three days. The fever is between 101 to 103 taken under the arm. He has runny nose for 7 days and cough for 4 days. He has no rashes, diarrhea, vomiting, headache, belly or back pain, and pain on urination. His appetite is reduced although he is drinking. He urination is normal. When the fever is down, he plays ball.
Past History: He had no serious illnesses before and never been hospitalized. The only illness he had are common colds.
Family History: His sister had colds 4 days ago.
Environment History: There are several children in the school who had strep throat in the past week.
Immunization History: He is complete with his shots.
Allergy History: He had rashes while taking Amoxicillin.
This is an example of a short but complete Clinical History. It would help a doctor if the parent prints this history and bring it to their doctor.
Giving a complete Clinical History can help a doctor figure out the diagnosis with ease.
The main purpose of the Clinical History is to help a doctor figure out the diagnosis as soon as possible so proper tests or treatment can be started as soon as possible. Now that you know how to give a good Clinical History, your child’s doctor will love your visit.
If one of the parent can’t bring the child to a doctor, a friend or close relative should bring a letter from the parents to give the doctor permission to treat child.
