BELLY PAIN IN A YOUNG CHILD

A 3.5 year old boy was brought at an ER because of persistent fever and abdominal pain.  This boy had been having fever for about one month. The mother is frustrated that in spite of two visits to the ER, the diagnosis of her son is still unknown.

She said that the fever is almost daily and the belly pain at night is so severe that she if fearful that her son might die. The belly pain is at the lower abdomen and it occur more often late at night, lasts about two hours, and spontaneously go away. During the day, her son is playful and seems normal.  His appetite is good and he has no diarrhea, vomiting, changes in stool pattern, pain on urination, back pain, cough, and rashes.

In spite of severe belly, his physical examination was within normal. He was playful in the daytime.

The first time he was seen at the ER he was diagnosed to have gastroenteritis and was given sulfamethoxazole (TMP/SMX).  On the second visit stool test showed Clostridium difficile toxin and Entamoeba coli.  He was given metronidazole.
