If your child has a Head Ache please print and answer these questions before seeing your clinician:
1. How long has been the headache?
2. How many headache does your child get in one month?
3. How long is each of the headache attack? How many minutes is the shortest and longest attack?
4. How severe is each attack? (Continues normal activity, Stops activity, Lies in bed.)
5. Is your child normal in between headache attacks?
6. What medications have you given for the headaches? (Which one helped most?)
7. Where is the headache located? (Front, sides, top, back)
8. Is there migraine in the family or frequent headaches?
9. Is there warning signs like flushing or bright lights before the headache?
10. What triggers the headache?
11. Is there nausea and vomiting?
12. Is there car or sea sickness?
13. Is there stress in school or at home?
You child's clinician might ask more questions depending on your answers.
