| How to Give a Good Clinical History |
| The clinical history provides at least 85% to 90% of the diagnosis especially in chronic conditions. Most experienced clinicians can make a reasonable diagnosis just by asking at least five to ten focused questions. The physical examination and laboratory tests are used to confirm their top diagnosis. However, most childhood illnesses do not need any tests. The following questions will aid in the faster diagnosis especially if your clinician is not using a "smart computer" software. If your child has Sore Throat, please print and answer these questions before seeing your clinician: 1. How many days is the sore throat? 2. Is there a fever? How many days is the fever? How high is the fever? 3. Is there runny nose? How many days is the runny nose? 4. Is there a cough? How many days is the cough? 5. Is there tiredness? How many days is the tiredness? 6. Is there abdominal pain? 7. Is there rashes? 8. Is there an ear ache? 9. Is there enlarged glands in the neck? 10. Is there pain in the chest?
