| How to Give a Good Clinical History |
| The clinical history provides at
least 85% to 90% of the diagnosis especially in chronic conditions. Most
experienced clinicians can make a reasonable diagnosis just by asking at
least five to ten focused questions. The physical examination and
laboratory tests are used to confirm their top diagnosis. However, most
childhood illnesses do not need any tests. The following questions will
aid in the faster diagnosis especially if your clinician is not using a
"smart computer" software. If your child has an Ear Ache, please print and answer these questions before seeing your clinician: 1. How many days is the ear ache? 2. Is there a fever? How many days is the fever? How high is the fever? 3. Is your child pulling at the ears? 4. Is there a runny nose? How many days? 5. Is there a cough? How many days is the cough? 6. Has your child been swimming in the past few days? 7. Is there ear discharge? 8. How many ear infections has your child had? 9. Is your child exposed to smokers? 10. When was the last time your child had an antibiotic?
