1. When was the enlarged gland noticed? (in days, weeks)

2. If there fever? What is the highest fever?

3. Is there pain at the enlarge glands?

4. Is there any cat or your child has been scratched by a cat?

5. Is there runny nose, sore throat, or headache?

6. Is there tooth or jaw pain?

7. Is there any scab, scratches, or lumps at the scalp?

8. Is there any rashes?

9. Is there any contact with Tuberculosis?

10. Is there a travel out of their town or state?

11. Is there poor appetite and weight loss in the past month?

12. Is there tiredness or fatigue?
