If your child has Abdominal Pain please print and answer these questions before seeing your clinician:

1. Since when was the belly pain? Is it a first episode or is it recurrent?                                      

2. Where is the pain located? (Pain at umbilicus is usually benign.)

3. Is there fever?                                                                                                           

4. Is there vomiting, diarrhea, constipation, back pain, etc.?        

5. If with diarrhea, describe the stools?

6. Has there been a recent travel or any similarly sick family member?                            

7. Are there rashes? (strep throat has rashes at upper chest).

8. Does the child have any food allergy?                        

9. Has there been injury lately?  (Trauma, spleenic rupture, hematoma)

10. Is there migraine or Crohns in the family? (Consider abdominal migraine.)
