A mother of a 2 month old boy called their physician because she is concerned that her son might be having seizure. A week before this episode, the baby was seen at a tertiary medical center and was diagnosed to have gatroesophageal reflux (Acid reflux; GERD). The mother was instructed to give ranitidine (Zantac) and metoclopramide (Reglan).

The mother reported that the baby is not acting right. He has stiffening of all the extremities, unusual cry, and abnormal responsiveness. A neighbor, who is a nurse, saw the baby during these episode and thought that the baby has seizures. The nurse also thought that the baby’s breathing is irregular and his color is pale.

The mother brought her son to the emergency room. On arrival at the ER, he was unresponsive, pale, and stiff. His temperature and blood pressure were normal. His heart rate was 210 beats/minute, respiration was 32/minute, and oxygen (pulse oximetry) was 88%.
